Consent for Surveillance Testing

Gila Crossing Families,

Next Friday, March 18, we will begin surveillance testing here on campus for thoise students attending in-person.

As a reminder, Executive Order #13 from Governor Lewis sets the expectation that we do periodic surveillance testing for all students attending in-person, in Part 16, section (c):

(c) the school provides surveillance testing in coordination with and upon recommendations from the Tribal Health Department and COVID-19 Task Force

In order for the testing to be done, and the results to be shared with us, we must have a signed Release of Information for EVERY student attending in-person. For example, if you have three students attending in-person, we will need 3 releases, one for each student.

If no release is signed and on file, and your student is not tested here on campus on the 18, you will need to show a lab-analyzed negative test for each student before continuing in-person learning.

THIS RELEASE OF INFORMATION FORM WILL BE SENT HOME WITH STUDENTS ON MONDAY AFTERNOON, AND WILL GO OUT ON BUSES TUESDAY MORNING.
