The Gila River Behavioral Health School Counseling Staff would like you to take the opportunity to take the following grade-level survey with your students. This is a survey that would be best done WITH your student.

Back to School BHSC Student Mental Health Survey 2022
GRHC’s Behavioral Health School Counseling Program in collaboration with the Community Schools, would like your assistance and permission to do a Back to School Student Mental Health Survey with input from both you as the parent/guardian and your student(s). The survey is to gather information about the COVID-19 pandemic’s effect on your child’s social and emotional wellness. This survey requires parent consent for student participation, and we ask that you as the parent/guardian also fill out a portion of this survey and assist your student with their portion of the survey questions. The information gathered will be used to help the BHSC program and the school to provide services and support for students and families. Your responses will be combined with all responses received to help provide the services students need. Individual responses will be kept confidential. There is a section for you the parent/guardian to fill out and a section for your student to fill out themselves. Please Complete Within One Week of Today’s Date: April 11
